Credential Check
All credential check sheets must be generated from Cybersport.
Background Check (Online Form)
Must be completed by any volunteer or employee who has routine access to children. Coaches, manager, referees, District Personnel, Locker room monitor, Board Member, Tournament Staff, etc.
Canadian Tournament Travel Permit
Must be filled out and submitted prior to participating in any events in Canada
